June 01, 2010, No. 2297-VI (as amended), I do hereby give to Arterium Corporation an unambiguous consent for processing* of my personal data under the terms and conditions defined by the Ukrainian legislation, in particular: name, phone number, e-mail, which confirm my identity, in card files and/or by using information and telecommunication systems of personal databases of Arterium Corporation for the purposes of implementing relations in the field of information security, relations in healthcare, providing implementation of these laws and other internal documents of the Company rights and obligations in the field of information exchange.

You may process my personal data pursuant to the above-stated purpose, particularly to transfer my personal data to any third parties, with no further permission or further notice of such processing by Arterium Corporation.

*Processing of the Personal Data means any operation or set of operations which is performed by any person on personal data, whether or not by automatic means, and includes the collection, recording, organization, preservation, storage, alteration, extraction, use, transmission, dissemination or any other form of disposal (including third parties), connection or combination, blocking, depersonalization or deletion.
